Question 1(Required)

I confirm that I hold a full current workplace first aid certificate the covers unit standards 6400,6401,6402 or an equivalent current Refresher certificate.

Question 2(Required)

As I completed my current certificate with another provider, I confirm that:

• BEFORE beginning my online modules, a copy of my certificate has been sent to Actions For Survival to verify the unit standards;

• I have received confirmation that I can proceed with the Revalidation/Refresher First Aid training as my certificate is valid;

• I understand that I do not need to send a copy of my current certificate if this was completed through Actions For Survival – I just need to tick the box.

Question 3(Required)

I completely understand and agree that if my current certificate is invalid, and I complete the online learning for the Revalidation/Refresher training, that I will not be eligible to do the practical skills-based assessment. If I wish to complete a certificate, I will need to complete the online learning for the full course.

Question 3(Required)

I understand that to receive my First Aid certificate, I must:

1. Complete the online learning, and

2. Attend a practical skills-based assessment to demonstrate competency in line with the first aid training requirements.
